Our Test Beds programme tests combinations of digital technologies with pathway redesign in a clinical setting with real patients and harnesses the potential of innovations to help transform the way in which healthcare is delivered; empowering staff to deliver care that improves the quality of life for patients and carers.  

To achieve this we are working in partnership with a number of innovations comprising of diagnostics, smartphone applications and management tools which are already making dramatic contributions to people with long-term conditions.

Administrators will be supported to use digital pathway tools TickerFit and DrDoctor to support patients to change their lives

TickerFit - a tool to support patients post cardiac event with exercise, education and medication management

Cardiac rehabilitation is being offered to individuals who have suffered from heart failure. We are working with Barts Hospital to pilot an alternative rehabilitation method, TickerFit, a tool to support patients post cardiac event with exercise, education and medication management.

WHAT WILL HAPPEN DURING THIS PILOT

Step 1 – Prescription of App

Your healthcare professional will talk you through your cardiac rehabilitation programme. The TickerFit app will be offered as part of your cardiac rehabilitation programme if it will be beneficial for you. Your healthcare professional will provide you with information about the app and assist you in setting up your profile.

Step 2 – Personalise your App

Based on your needs, the healthcare professional will personalise your exercise and educational plan.

They will take base measurements including blood pressure, pulse and BMI.

Step 3 – Mid Point Review

Four weeks into your plan you will be invited to an appointment to have your base measurements taken again. Any potential changes to medications and your programme plan would also be discussed during this appointment.

Step 4 – Check-In

Your healthcare professional will contact you after your midpoint review, but before your final appointment to discuss how you are getting on with the plan

Step 5 – Final Check-In

We hope the data collected from this pilot will indicate:

  • How many people use the technology
  • What the outcomes of using the technology is
  • Whether the service is good value for the NHS, both by way of a patients experience and the cost of running the service.

We will train domiciliary care staff to use digital innovations to become the eyes and ears of the health system

Whzan - a technology to help you stay well and comfortable at home

Whzan - a health monitoring technology designed to help your Bluebird Care team in supporting decisions if you may show signs of illness

We are working with Bluebird Care to help you stay well and comfortable at home and we will do this with a tried and tested healthcare technology called Whzan Digital Health. 

Whzan is a health monitoring technology designed to help your Bluebird Care team in supporting decisions if you may show signs of illness. A Whzan computer enables your Bluebird carer to record your temperature, blood pressure and pulse to detect signs of a potential illness, or infection so it can be treated quickly and effectively.

WHAT WILL HAPPEN DURING THIS PILOT

Step 1 – Initial Advice

The Bluebird team will talk with you about the pilot

Step 2 – Check Up

One of your visits each week will be extended to an hour with your carer. They will provide all of the care you usually receive and take your blood pressure, blood oxygen and temperature using the Whzan kit. This is a quick, easy and painless process.

Step 3 – Follow Up

These results will be recorded by Whzan and can be seen by your GP. If your GP believes you are unwell, your carer can help to co-ordinate the right course of action.

Healthy.io - a tool to make sure that you stay well and remain comfortable at home

Healthy.io - a technology designed to spot patient deterioration and better manage medication and patient care

We are working with your care provider, The Lodge Group to make sure that you stay well and remain comfortable at home. We are doing this through the delivery of two different technologies, one of which is a urine dipstick test called Healthy.io

Urinary tract infections (UTIs) are incredibly common amongst older people living with complex conditions. You may have had a UTI in the past, which resulted in you feeling very unwell, and perhaps even led to an admission to hospital. However, when identified early, UTIs are easy to treat with a course of antibiotics. This can enable you to stay well at home.

Healthy.io’s urine dipstick test has the potential to allow your carer to quickly and easily detect whether you are suffering from a UTI, through simply taking a picture of a urine dipstick on their mobile phone.

WHAT WILL HAPPEN DURING THIS PILOT

Step 1 – Initial Advice

Your carer will talk to you about the programme. They will advise how you could benefit from Healthy.io

Step 2 – Reminder

If you are suspected to be suffering from a UTI at any point over the next year, your carer will remind you of the pilot and your consent to use Healthy.io.

Step 3 – Check Up

Your carer will then take your vital signs (for example, your temperature). They will do this through using a piece of kit called Whzan, which you have also received information about. They may also ask you a few questions, to find out a bit more about how you are feeling.

Step 4 – Urine Sample

They will then ask for a sample of your urine, so they can undertake the dipstick test.

Step 5 – Treatment

The carer will send these results to your GP, who will be in touch to organise antibiotics or other appropriate treatment if you do have a suspected infection.

We will support healthcare assistants and other non-clinical staff within primary care in Barking & Dagenham to become digital prescribers

Sleepio - to test the use of a Smartphone app in the management of insomnia

Sleepio - a smartphone app designed to help adults who have difficulty falling asleep through the night

We are working with GP practices in Barking & Dagenham to test the use of a Smartphone app, Sleepio to manage insomnia and we would like you to be involved. Sleepio is a 6 week digital programme designed to help people with poor sleep and can help to relieve depression and anxiety.

The programme is delivered by The Prof, your personal sleep expert and uses a variety of proven methods including sleep diaries, sleep hygiene, sleep restriction, and other Cognitive Behavioural Therapy techniques.

WHAT WILL HAPPEN DURING THIS PILOT

Step 1 – Prescription of App

You will receive information about Sleepio from your GP

Step 2 – Registering with Sleepio

You will create an account on the Sleepio website.

Step 3 – 3 week check in

You will be contacted by a healthcare assistant from your practice over the telephone to discuss your progress with Sleepio.

Step 4 – 6 week follow up

You will need to make an appointment with your GP following completion of the 6-week Sleepio programme. This is to see if your sleep has improved.

Liva a digital health coaching programme designed to help patients with newly diagnosed type 2 diabetes make lifestyle and behavioural changes to improve their health and wellbeing

Liva - a digital health coaching programme designed to help patients with newly diagnosed type 2 diabetes make lifestyle and behavioural changes to improve their health and wellbeing

Working with GP practices across Barking & Dagenham, we are testing Liva, a 9 month health lifestyle e-coaching programme for newly diagnosed type 2 diabetics. The Liva app lets you track diet and lifestyle factors of your choice.

Once on the programme you will be assigned a personal coach who will discuss and set your individual health goals over the telephone. You can then use the app to log and track your progress towards these goals. You will work with the same coach for 9 months through the Liva app.

WHAT WILL HAPPEN DURING THIS PILOT

Step 1 – Get Informed

You will receive information about the digital app (Liva) from a member of your GP practice team like your health care assistant, nurse or GP.

Step 2 – Initial Checks 

If you’d like an e-coach through Liva you will need to have had an HbA1c level and weight check within the last 3 months. If it’s been longer than 3 months since your last check you will need to schedule this at your surgery.

Step 3 – Book at appointments

Once your Hba1c and weight is up to date you can schedule an appointment with your HCA or nurse to get access to Liva. 

Step 4 – Deciding health goals

Once you register with Liva, you will be matched with a health coach. You will meet your health coach online to agree health related goals. They will monitor progress towards your goals.

Step 5 – 3 month check in

You will be contacted by your practice and health coach to make appointment at your GP practice to have an HbA1c level check and your weight taken. Before attending this appointment you will need to collect a blood form from your GP Practice.

Step 6 – 9 month check in

You will be contacted by your practice and health coach to make appointment at your GP practice to have an HbA1c level check and your weight taken. Before attending this appointment you will need to collect a blood form from your GP Practice.
